Community Development Workers – support people to create plans for their community that address local needs and issues; usually related to recreation, health, housing, employment and welfare matters.
East Surrey Place – a group of organisations with a formal alliance agreement in place, working in the East Surrey area to plan and deliver a range of health and care services to improve the health of our population.
GHT – Growing Health Together
GP Federation – Group of Primary Care providers which forma single entity to deliver services for their combined patient communities.
ICB – Integrated Care Board (Commissions health services as is necessary to meet the requirements of its population)
ICS – Integrated Care System – Local partnerships that bring health and care organisations together to develop shared plans and joined-up services. There is one ICB in every ICS.
Local Authority – organisation responsible for all public services and facilities in a particular area.
Neighbourhoods – Neighbourhood health aims to create healthier communities, helping people of all ages live healthy, active and independent lives for as long as possible while improving their experience of health and social care and helping their independence.
NIHR – National Institute for Health and Care Research
PCN – Primary Care Network (GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas)
